The relevant information is as follows:

first, the main achievements of safety evaluation since 2011

since 2011, the safety supervision departments at all levels have taken strong measures to strengthen the supervision of safety evaluation institutions, and the safety evaluation work in our province has shown a healthy and orderly development trend< (1) the safety evaluation supervision system has been further improved. Since 2011, the provincial administration of work safety has successively issued the "layout plan for the development of safety evaluation institutions in Hubei Province" (hereinafter referred to as the "layout plan"), the "regulatory measures" and the "assessment rules", further improving the supervision system of safety evaluation institutions in Hubei Province. First, in accordance with the basic idea of "overall planning, reasonable layout, total amount control and moderate development", and in line with the principle of reasonable regional and industrial distribution, balanced number of industries and institutions in the region, complementation of institutions and industries in adjacent regions, and Realization of no blank area and no blank industry, the "layout planning" is put forward to solve the problem of unreasonable layout of safety evaluation institutions in the whole province, The development is not balanced, the function is not complete, and the business scope is too concentrated, which leads to disordered market competition and irregular evaluation behavior. Second, the newly issued "supervision measures" has refined the qualification conditions of institutions, clarified the approval links and procedures, differentiated the responsibilities and tasks of supervision of safety supervision departments at all levels, and solved the problems of establishment of institutions, too large flexibility of approval, difficult to grasp, unclear responsibility, unclear tasks and incomplete mechanism. 1n view of the non-standard evaluation behavior, on the basis of sorting out the punishment clauses of illegal evaluation behavior in laws and regulations, this paper focuses on solving the problems of management blank, insufficient illegal punishment, bad influence of violation of professional standards and so on. The third is to implement "double seven items" assessment for safety assessment institutions, that is, seven necessary standards are negated if one is missing, seven management standards are refined into 52 sub items, 100% assessment is implemented, and dynamic management is implemented, which has changed the past practice of three years' batch management of qualification< (2) the supervision mechanism of safety evaluation has been further improved. Through the establishment of mechanisms such as responsibility review of safety evaluation report, interview of person in charge of safety evaluation organization, filing of safety evaluation organization in other provinces and online disclosure of relevant information of safety evaluation report, the establishment of responsibility awareness of safety evaluation organizations has been effectively promoted. Through a series of activities, such as comprehensively cleaning up the basic information of safety evaluation institutions and evaluators, announcing to the society, carrying out special management of safety evaluation institutions' supervision and professional behavior, and training of safety evaluation personnel, the professional quality of evaluators and the technical service quality of safety evaluation institutions have been effectively improved< (3) the technical support capacity of safety assessment institutions was further enhanced. At present, there are 5 safety evaluation institutions with grade a qualification and 13 safety evaluation institutions with grade B qualification in our province. There are 411 safety assessors registered in the above organizations, of which 98 are newly added and 227 are registered safety engineers. The office space, fixed assets, registered capital, general equipment, evaluation personnel and other qualifications of each evaluation institution are maintained in good condition. The improvement of technical service conditions of safety evaluation institutions, the improvement of comprehensive quality of evaluation personnel, and the strengthening of daily supervision and management have led to the standardization of evaluation activities and the improvement of evaluation quality. 1n 2011, the provincial safety evaluation institutions completed 1493 safety evaluation reports, pointed out 5941 potential safety hazards, and put forward 9200 rectification measures and suggestions, which made positive contributions to the continuous improvement of the provincial safety production situation< Second, the main problems are as follows: through the comprehensive inspection and assessment of the safety evaluation institutions and the investigation of related problems, we find that the qualification conditions of the safety evaluation institutions in our province are not stable, and the internal management needs to be strengthened; Pay more attention to economic benefit than risk control; Emphasis on the number of reports, light process control and other issues< (1) process control needs to be strengthened. The training and learning of process control documents by some safety evaluation organizations are not in place, and the evaluation personnel, especially the quality process control management personnel, do not have a good grasp of the content of process control documents, which leads to non-standard process control and low quality evaluation report. A small number of safety evaluation institutions neglect the management of evaluation process control, which makes it difficult to play the role of audit and guarantee the quality of evaluation report. First, risk analysis is a mere formality. Risk analysis should be carried out before signing the project contract. However, the risk analysis of some safety evaluation institutions is not standardized after the signing of the project contract, and even some individual projects of a few safety evaluation institutions such as Huanggang Oriental safety inspection institute have not carried out risk analysis. Second, the internal audit of the evaluation report is not meticulous. 1n a few safety evaluation organizations, the internal audit of evaluation report is carried out by project team members, and the process control audit is carried out by non process control person in charge. Third, the signature of the evaluation report is inconsistent. The signature of the internal audit record of the same evaluation report is inconsistent with that of the signature form of the evaluation report; 1n the same safety evaluation organization, the signature of the same evaluation personnel on different evaluation reports is inconsistent, and the personnel who did not participate in the evaluation report audit also blindly signed. Fourth, the audit record of the report is not detailed. Some safety evaluation organizations only sign the audit record, and there is no audit content, or even an empty table. Some organizations have no treatment opinions and results on the modified content< (2) the updating and implementation of the standard system need to be strengthened. First, the standard system was not updated in time. The system of some institutions has not been modified in time according to the actual situation, the relevant content is old, and the pertinence and operability are not strong. Third, the implementation of the standard system is not effective. Some have established systems, but they have not been implemented seriously. 1n particular, the new "regulatory measures" and "assessment rules" issued by the Provincial Safety Supervision Bureau were not seriously publicized and implemented, and the relevant requirements were not implemented in place< (3) internal management needs to be strict. First, the file management is not standardized. Some archivists are not familiar with the contents and requirements of archives management, methods and processes, and their professional quality needs to be improved. Some institutions have not established file index and retained the draft of evaluation report for review. The evaluation report and image data of individual institutions are still kept in the hands of the evaluation personnel and have not been filed. Second, personal performance records are incomplete. Some institutions are not clear about the contents and requirements of performance statistics and reporting, have not established performance records and statistical mechanism, have not carried out detailed records at ordinary times, and the statistical reporting elements are incomplete. Third, the daily management is not in place. This is reflected in the lack of attention paid to the maintenance of the qualification conditions of the organization, the renewal of the labor contract and the employment agreement of technical experts, the payment of "four insurances and one fund", the filing of the evaluation report, and the failure to handle the performance records on time< (4) the evaluation fee is low. According to the guidance on charging of safety assessment industry in Hubei Province issued in 2009, all institutions failed to meet the requirements of charging guidance price, and some institutions' charges were significantly lower than the guidance price. Fourthly, the attachments of some evaluation reports are incomplete. Such as the lack of special equipment inspection report, lightning protection inspection report, safety facilities inspection report, effective drawings issued by the survey and design unit, etc< (6) the qualification conditions are not stable enough. First, the individual insurance payment is insufficient. The personal protective equipment and computers of individual organizations did not meet the requirements of one set for each safety evaluation personnel. Third, the number of technical directors is insufficient. The technical directors of individual institutions can not meet the needs of the declared specialty, and even the technical directors of individual institutions are not qualified< Safety evaluation is the basic work of strengthening safety production, the pre means of safety production process control, and the important basis for safety regulatory agencies to administer according to law.
