At about 17:30, Zhou and Hao informed Master Liu to replace the short flange joint in the metering tank. Master Liu instructed the workers to open the manhole cover of the propylene oxide metering tank without obtaining the work permit for entering the confined space. At that time, the workers did not take corresponding safety measures, and suffocated when they entered the tank through the manhole.
